The QTc calculator is based on heart rate (pulse rate) in beats per minute and QT interval, either in seconds or miliseconds.
■ Heart rate is defined as the number of subsequent contractions of the heart during one minute, in which blood is pumped from the ventricles to the arteries.
■ The QT interval is a cardiologic term used for the time in the heart’s electrical cycle between the start of the Q wave and the end of the T wave. This is measured in seconds or in miliseconds, the latter for more precision.
QTc formulas
The corrected QT interval is calculated based on the following:
■ Bazett’s formula: QTc = QT/√(RR in seconds)
■ Fridericia’s formula: QTc = QT/(RR0.33)
■ Framingham’s formula: QTc = QT + 0.154(1-RR)
■ Hodges’s formula: QTc = QT + 1.75(HR — 60)
According to the ACC/HRS:
■ Normal values are lower than 450 milliseconds for men and below 460 milliseconds for women;
■ Any QTc value greater than 500 milliseconds is abnormal;
■ Any QTc value lower than 340 milliseconds is abnormal and may indicate short QT syndrome.
After one comparison study, it was concluded that Fridericia and Framingham correction formulas have the best rate correction and significantly improved prediction while Bazett’s formula overestimated QTC prolongation.
Abnormally high or low QTc
Values lower than normal may be caused by short QT syndrome which is a condition that in turn leads to arrhythmia. This is not considered to have to do with any underlying structural anomaly of the heart and can appear at any age.
A shorter QT interval basically means that the heart takes less time to relax between contractions. This can lead to syncope, dizziness and even complicate to cardiac arrest and sudden death.
Prolonged QT syndrome is also indicative of an arrhythmia type and is characteristic by moments in which the heart suddenly beats faster for no particular reason.
This increase in contraction frequency that has not been supported by other physiological adaptation can lead to impaired tissue perfusion of the brain.

How does this QTc calculator work?
This is a handy health tool that can estimate the QT corrected interval by using the heart rate expressed in beats per minute and QT interval expressed either in seconds or milliseconds. The QT values can be obtained from the ECG test.
This QTc calculator is designed to show the QT corrected interval for heart rate extremes because it returns the estimations by 4 different equations as presented below:
QT corrected interval:
■ by Bazett’s formula: QTc = QT/√(RR in seconds)
■ by Fridericia’s formula: QTc = QT/(RR^0.33)
■ by Framingham’s formula: QTc = QT + 0.154(1-RR)
■ by Hodges’s formula: QTc = QT + 1.75(HR — 60)
Where:
RR interval = 60 / HR
HR = Heart rate in beats per minute.
Moreover it returns the QT corrected interval expressed in both seconds and milliseconds.
As agreed upon by ACC / HRS the normal QTc interval is below 450 milliseconds for men and below 460 milliseconds for women.
No gender specific, any QTc greater than 500 milliseconds is considered highly abnormal, while any value of QTc smaller than 340 milliseconds may indicate short QT syndrome.

For a heart rate/ Pulse of 72 beats per minute and a QT interval of 0.42 seconds the result is:

■ QTc Interval by Framingham’s algorithm = 0.446 sec OR 446 msec

What is the short QT syndrome?
This is a condition that can cause arrhythmia which is a disruption in the heart’s normal rhythm because the QT interval shortening means that the heart takes less time to recharge/ relax between beats but there is no underlying structural anomaly of the heart.
This is a relatively new discovery of the 21st century medicine and there haven’t been numerous cases documented. It can be detected through EKG (electrocardiogram) that measures the electrical activity of the heart.
This condition that appears at any age, if left untreated leads to syncope which is fainting, feelings of dizziness and even to cardiac arrest and sudden death. On the other hand there are people, generally healthy that have shortened QT but don’t display any symptoms.
What if the QT is higher than normal?
The long QT syndrome is an uncommon condition, also put under arrhythmias and can pose a serious threat as the electrical activity of the heart is disrupted. There are also some individuals that have QT intervals longer but don’t develop serious arrhythmia while others experience moments in which their heart suddenly beats faster for no particular reason and this disruption of rhythm leads to the brain not being oxygenated properly and then fainting.
It is discussed that there is an inheritance pattern for this anomaly and that there are higher chances for it to appear to individuals that have cases of heart disease in the family.

где:
QTc — корригированная (относительно частоты сердечных сокращений) величина интервала QT, относительная величина.
RR — расстояние между данным комплексом QRS и предшествующим ему, выражается в секундах для формул Базетта и Фредерика, и в миллисекундах для формулы Саги.
Формула Базетта не вполне корректна. Отмечена тенденция к излишней корректировке при высокой частоте сердечных сокращений (при тахикардии), и недостаточная корректировка при низкой (при брадикардии).
При удлинении интервала QT >500 мсек повышается риск развития фатальных нарушений ритма, в том числе полиморфной (веретенообразной) желудочковой тахикардии, которая несёт непосредственную угрозу жизни пациента.
QT считается чрезмерно укороченным при значении <340 мсек.
Для ЧСС ниже 60 или выше 100 ударов в минуту определение корригированного значения интервала QT должно быть произведено по формуле Sagie (Framingham).
Нормальные значения корригированного QT согласно рекомендациям ACC / HRS: 340-450 для мужчин и 340-460 для женщин

Интервал QT измеряется от начала зубца Q (или R при отсутствии Q) до конца зубца Т. Этот интервал отображает полную систолу желудочков: и их деполяризацию (комплекс QRS), и реполяризацию (зубец Т).
Особенности измерения QT и QTc и их клиническое значение
- Для определения QT измерьте длительность от начала комплекса QRS до конца зубца Т, который определяется пересечением касательной к максимальному наклону Т и изолинии (красная точка на схеме ниже). Оптимальные отведения для измерения — II стандартное и V5-V6.
- Не существует какого-то одного нормального значения QT: интервал постоянно изменяется и становится короче при повышении ЧСС (как и все остальные интервалы и сегменты ЭКГ).
- В то же время, при снижении частоты ниже 60 ударов в минуту интервал QT перестает удлинятся.
- Для «нормализации» показателя используется т.н. «корректированный QT» (QTc) — рассчитанный на основании QT и частоты сердечных сокращений.
- При расчете используется RR между измеряемым и предшествующим (!) комплексом QRS.
- Нормальный QTc = 340-450 мсек у мужчин и 340-470 мсек у женщин.
- Удлинение QT ассоциировано с повышением риска фатальных аритмий — желудочковых тахикардий, в т.ч. и типа «пируэт» (т.н. Torsades de Pointes).
- Укорочение QT — группа очень редких наследственных синдромов, также приводящих к внезапной смерти вследствие пароксизмальных желудочковых аритмий.
Формулы для измерения QTс
Чаще всего используется формула Базетта — именно она «вшита» в программу большинства кардиографов. Эта формула была придумана в 1920 году и по ней наработана самая большая доказательная база. Сейчас придуманы и изучаются более эффективные альтернативные формулы, часть из которых описана ниже.
- Формула Базетта: QTc = QT/√(RR)
Используйте формулу Базетта с осторожностью: при ЧСС менее 60 или более 100 в мин. погрешность становится значительней. Формула «завышает» QTc, приводя к гипердиагностике удлиненного QT.
- Фрамингемская формула: QTc = QT + 0,154(1-RR)
Используется как обязательная альтернатива формуле Базетта при ЧСС ниже 60 или выше 100 в минуту, хотя согластно данных последних исследований превосходит предыдущую формулу в рутинных ситуациях [2].
- Формула Фридерика: QTc = QT/(RR)0,33
Похожа на формулу Базетта, с той лишь разницей, что QT делится не на квадратный, а на кубический корень из RR. Обладает большей точностью, чем формула Базетта, при частотах выше 60 уд. в минуту [1], также показала большую точность, чем остальные формулы, при оценке QTc у больных с фибрилляцией предсердий [3].

What is QT interval?
The QT interval is an electrocardiogram measurement to assess some of the characteristics of the electricity of the heart. It is measured as the time from the beginning of the Q wave to the end of the T-wave and close to the time from when the chamber of the heart begins to contract after completing relaxation. An unusually short or long interval is correlated with the chance of creating abnormal heart rhythms and immediate death of the heart. QT interval abnormalities can be caused by genetic disorders such as long QT syndrome, certain drugs, and blood salt disorders such as hypokalaemia or hypothyroidism due to hormonal disorders.
How QT interval is measured?
The QT interval for serial ECG evaluation is most often measured in lead II, while leads I and V5 are comparable to lead II. In order to measure the QT interval, leads III, V1 and aVL are usually avoided.
ecise QT interval calculations are subjective because the end of the T wave is not always well defined and gradually joins the baseline. The QT interval in an ECG complex can be manually measured using various methods, such as the threshold method, where the end of the T wave is defined by the point at which the T wave component merges with the baseline isoelectric. Or the tangent method in which the T- wave end is defined by the tangent line intersection coming to the isoelectric baseline from the T wave at the maximum drop point.
Why Bazett's formula?
The most commonly used formula for QT correction is the Bazett formula, named after Henry Cuthbert Bazett( 1885- 1950), a physiologist who invented it to calculate the heart rate-corrected interval (QTcB) of QT. The formula of Bazett was based on the observations of a 1920 study. The correct mathematical form of Bazett's formula is:
Where QTcB is the heart rate QT interval, and RR is the interval between the onset of one QRS complex and the onset of the next QRS. This mathematically correct formulation returns QTc generally in milliseconds in the same units as QT. This formula, in some popular forms, measures QT in milliseconds and RR is measured in seconds, often derived from the cardiac frequency (HR) as 60/HR.
